Bergen Hypertension & Renal Associates is in network with the following health plans.When calling for your appointment, please let the receptionist know under which plan you are covered. Some plans require referral forms from your primary care physician, please verify with your insurance carrier. If this is the case, it is essential that the referral form is properly filled out and is brought to our office at the time of the visit. If you do not belong to a health plan we participate in, out-of-network benefits usually apply. An out-of-network payment is expected at the time of your visit. We file all insurance claims for you. Please speak to our billing supervisor if you have any questions on insurance coverage.

A valid Insurance card is required at each visit. All new patients must show a valid ID at time of their appointment.
All copays are due at time of service. This arrangement is part of your contract with your insurance carrier. We accept Master Card, Visa, Cash & Checks.
